We register new patients from an area covered by a one mile radius from our practice building. If you would like to register to our practice please complete our online registration form.
When you register, we will ask you to make an appointment to see the practice nurse (or doctor if you are on regular medication) for a new patient health check. This will ensure that our records are accurate and all routine checks to promote your health and well being (such as blood pressure/smear tests/mammograms) are up-to-date.
How do I become a proxy for another patient?
If you would need to be a proxy for another patient, please visit the surgery and fill in our Proxy Request form.
Accountable GP
When you are registered with our surgery, you will be be appointed an accountable GP, also known as your Usual GP. This is the GP that will appear on your medical records. It is assigned at random and you do not need to see that GP when you want an appointment. If you would like to change your accountable GP, please speak to reception and we will change it on request.
